In the case of accidental or wrongful relocation of consciousness, especially where the incident is felt and recognized, there are ways to identify the consciousness in question. 

MTO, if one exists can lead the person who recognizes the MTO to find and keep or gain back the consciousness. 

Birth patent, which is birth certificate identified originating municipality, hospital, and person. The social security card does this as well, or passport and identifying documents from country of origin. 

Replenishing conscious connective body with emphasis on DNA, blood type markers, and molecularly identified fingerprints is also a way to manage this, which is highly dependent on routine, activities of a nature which have been performed by the person in question and are again.
